Growing Minds Youth Center welcomes referrals from schools, social workers, care managers, therapists, healthcare professionals, and community organizations seeking supportive youth wellness and social-emotional programming for children and adolescents.

Program Fit
Programs are currently virtual, free to families, and reviewed based on the child or youth's age, program availability, and participation fit.
Caregiver Awareness
A caregiver must know about the referral and agree to be contacted. Growing Minds will review the submission and contact the caregiver when appropriate.
No Enrollment Guarantee
Referral submission does not guarantee enrollment, acceptance, or a specific response time.
Standard Registration
Parents and caregivers who are not working through a professional partner should continue using the standard registration process.
Privacy and Confidentiality
Submit only what is needed for referral review.
Please submit only the information necessary for Growing Minds Youth Center to review the referral and contact the caregiver.
Do not submit Social Security numbers, insurance identification numbers, immigration records, detailed medical records, psychiatric records, or other unnecessary sensitive information.
Information submitted through this form will be available only to authorized Growing Minds administrators involved in referral review.
